Bible Knowledge

1. List home study and other books read.
2. Record Sunday School, Bibles Studies, and Worship services where books were taught.
3. Show all conferences, camps, or tapes and subjects covered.

Christian Growth

This is meant not to measure your sanctification, but to simply address areas of growth achieved or needed.

1. Comment on your personal prayer life, devotions, and personal worship.
2. Any areas in your personal life, devotions, and personal worship.
3. Testimony as to what the Lord is doing in your life this past year.

Christian Service

1. Bible classes, Bible clubs, or Sunday School classes taught.
2. Witnessing to the lost.
3. Service within the church-yard work, ushering, music ministry, nursery, cleaning the church, other Christian work outside the church, rides given to church, ministry to the needy, nursing home, and/or visits to sick or shut in.
4. Testimony in the community or neighborhood-giving blood, volunteer work, etc.
